Proxy formatter

Notes
Mixed input is fine
Lines in different formats can be mixed freely; each is parsed on its own terms and normalised into the output shape you pick.
Deduplication
Duplicates are matched on host, port and username, so the same address with two credential pairs is kept as two entries.
